Sexual Harassment Reporting Form
The should be used to report any incident involving Rockhurst's Sexual Harassment Prevention and Response Policy (including sexual harassment, sexual violence, domestic violence, dating violence, stalking, or any other form of sex discrimination). Please note that all staff, faculty, and administrators (excluding those working in a confidential capacity such as counselors, medical providers, and clergy or campus ministry staff when providing pastoral care) are required to disclose any reports of sex discrimination and may not make anonymous reports. 91Թ encourages all victims to make a report with law enforcement agencies and seek immediate medical attention at the local hospital, especially in instances of sexual violence.
Discrimination, Harassment and Retaliation Reporting Form
ճ is for reports of discrimination, harassment and retaliation on the basis of race, color, creed, religion, sexual orientation, marital status, national origin or ancestry, age, gender, disability, citizenship or intended citizenship status, veteran status or any other status protected by law (“protected status”). (Discrimination, harassment and retaliation on the basis of sex are governed exclusively by the University’s Sexual Misconduct Prevention and Response Policy)
Incident Reporting Form

Examples include drugs, alcohol (including amnesty), harassment, disorderly conduct, quiet hours, visitation, etc. If you have questions about whether a violation falls under this category, please contact the Assistant Director of Residence Life at 816-501-4126.
Concerning Behavior Reporting Form
ճ is for sharing concerns you have regarding the well-being of others on campus or concerning behavior that may not be considered a conduct violation but is worrisome nonetheless.
By sharing your concerns, you are helping to maintain Rockhurst as a healthy and safe campus environment. Once received, the information will be reviewed and you may be contacted for additional information. If you wish to make an anonymous report of concerning or worrisome behavior, the report will be evaluated and a determination will be made for what follow-up (if any) is necessary to address the concerns. You are strongly encouraged to provide your name and contact information to allow us to call or email you if we have additional questions regarding your report. Pregnancy or Parent Adjustment Request for Students Form
A student who becomes pregnant is strongly encouraged to notify 91Թ's ("Rockhurst") Title IX Coordinator as soon as possible. The Title IX Coordinator and the student can collaborate and develop an appropriate plan for the continuation of the student's education in light of the unique nature of Rockhurst's programs and any clinical requirements, as well as particular challenges the student may face while pregnant or when recovering from childbirth (e.g. missed classes, clinicals, make-up work, etc.). However, the choice to declare a pregnancy is voluntary, and a student is not required to disclose this information to Rockhurst.
Students with parenting responsibilities who wish to remain engaged in their coursework while adjusting their academic responsibilities because of the birth or adoption of their child or placement of a foster child or other extraordinary parenting responsibilities may request an academic modification period of up to two weeks beginning with the first six months of either the child entering the home or when the extraordinary circumstances arose.

Academic Early Alert
The may be completed for early alert notifications for the Student Success Network. Students who receive an early alert will be contacted and early interventions will be provided as appropriate by professional staff members.
